Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (NP/PA) to perform in-home health assessments across Idaho, with travel about 85% of the time. Live anywhere in the state. Flexible 8am-8pm schedule over a 7-day week; no on-call, weekends, or holidays.

Competitive base salary plus incentives and loan repayment or sign-on bonus. Responsibilities include comprehensive home visits, documenting diagnoses, and collaborating with primary care providers to close care gaps.

Qualifications

• NP certification (ANCC/AANP) in Family, Adult, Gerontology Primary Care.
• Active, unrestricted licensure in practicing state.
• Active driver's license and reliable transportation for home visits.

Responsibilities

• Conduct comprehensive in-home health assessments for Medicare Advantage and other members.
• Document diagnoses for care management and treatment planning.
• Coordinate with PCPs to address gaps in care and ensure follow-up.
