Job summary

Hueman is seeking Nurse Practitioners and Physician Assistants for a home-based assessment role. You will travel to patients’ homes to perform comprehensive, non-invasive physical assessments and review histories, ensuring accurate documentation and patient safety through medical reconciliation.

Role emphasizes patient education on wellness and preventative care, with prior licensed NP/PA credentials. This opportunity supports flexible scheduling and involvement in local healthcare networks,

Qualifications

• Master’s Degree OR commensurate experience and NP/PA license is required.
• Active State NP/PA license is required.
• Active National board certification is required.
• Current BLS certification is required.
• 12 months of experience as a paid advanced clinical provider is preferred but not required.

Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.

Education
