Job summary

Mental Health America of Los Angeles seeks a Licensed professional to serve as Personal Service Coordinator / ASW AMFT APCC for the Hope FSP mobile outreach team. The role focuses on assessments, crisis intervention, and continued support in outreach settings.

You will provide case management, coordinate housing and health services, and document activities to DMH requirements, working with homeless individuals to promote recovery and stability.

Qualifications

• Must be licensed or licensed-waivered as ASW/AMFT/APCC.
• Experience in outreach and case management is preferred.
• Knowledge of DMH and BHSA frameworks is a plus.

Responsibilities

• Complete diagnostic assessments and crisis interventions.
• Provide outreach and engage homeless individuals for program services.
• Coordinate housing, health care, and primary care referrals in the field.
