Job summary

Behavioral Health Connections, Inc. in Houston, TX is seeking a Mobile Assessor full-time evenings to join a dedicated team. The role conducts mental health assessments, triages patients to the appropriate care, and coordinates discharge planning for contracted facilities.

Required qualifications include a nursing degree or social work/psychology master’s, and active licensure (LPC/LMFT/LMSW/LCSW/RN). CPR may be required by partner hospitals. Strong crisis intervention skills are essential.

Qualifications

• Degree in Nursing or Master’s degree in Social Work, Psychology, Counseling, Behavioral Health or related field.
• Current license in state as LPC, LMFT, LMSW, LCSW, or RN.
• CPR current if required by customer hospital.

Responsibilities

• Provide mobile mental health assessments for contracted ERs/hospitals.
• Recommend appropriate level of treatment based on assessment.
• Triage patients to suitable services and document findings.
• Coordinate care with hospitals and facilities as needed.
