Job summary

Northwest Human Services (NWHS) in Salem, OR seeks a Mobile Medical Unit Driver / Community Health Worker to deliver care beyond the clinic walls. You will drive to outreach sites, set up essential equipment, and help patients access healthcare and services.

As a CHW, you will connect patients with healthcare and social resources, address social determinants of health, and coordinate care with partners. This full-time, non-exempt role emphasizes outreach and compassion.

Mobile Medical Unit Driver / Community Health Worker

Location: West Salem Clinic | 1233 Edgewater St NW, Salem, OR

Schedule: Full Time | Monday–Friday with rotating Saturdays and community outreach events

Status: Non-Exempt

Reports to: Community Outreach and Engagement Manager

As a Mobile Medical Unit Driver/Community Health Worker (CHW), you will help bring care, connection, and resources directly to people where they are.

This is a unique opportunity to combine mobile healthcare operations, community outreach, patient support, and care coordination in one meaningful role.

You won't simply drive a vehicle—you'll help bring the healthcare team to the community.

What you’ll do as a MMU Driver

• Safely drive the MMU to community locations and outreach events.
• Plan routes and adjust travel based on traffic, road, and weather conditions.
• Complete required vehicle inspections and maintain accurate vehicle and MMU records.
• Safely set up, operate, and power down MMU systems and equipment.
• Assist with loading, unloading, securing, and organizing medical supplies and equipment.

What you’ll do as a Community Health Worker (CHW)

You will work alongside patients, healthcare providers, care teams, and community partners to help identify and address barriers that can make it difficult for people to access healthcare and improve their overall well-being.

• Helping patients identify and address social determinants of health (SDOH).
• Connecting patients with healthcare, social services, and community resources.
• Conducting outreach to patients and helping them stay connected to care.
• Supporting patients who may experience barriers related to housing, transportation, healthcare access, behavioral health, substance use, or other social needs.
• Participating in care coordination and multidisciplinary team discussions.
• Responding to referrals and warm handoffs and completing appropriate documentation.
• Building relationships with community organizations and sharing resource information with the NWHS team.

What you’ll bring

Required Qualifications

• High school diploma or equivalent.
• Valid Oregon Driver's License and a driving record that allows coverage under the NWHS auto liability policy.
• Ability and willingness to safely operate an MMU or comparable large vehicle after completing required training.
• Current Basic Life Support (BLS) for Healthcare Providers certification consistent with American Heart Association guidelines, or ability to obtain it within the required timeframe.

Preferred Qualifications

• Community Health Worker certification.
• Experience in healthcare, human services, community outreach, transportation, vehicle operations, or a related setting.
• Experience working with underserved populations or individuals experiencing health and social barriers.
• Bilingual in English and Spanish or English and Russian and able to act as an interpreter a plus.
