Adara Home Health, Inc. is seeking a Licensed Practical Nurse to join our Minnesota team. The LPN provides comprehensive client care in home settings under the Case Manager and Clinical Supervisor, traveling to clients’ homes as needed.

This full-time role, Monday–Friday with on-call rotation, serves the Blaine area including New Brighton, St. Anthony, Roseville and nearby communities. Benefits include PTO, mileage reimbursement, 401k, tuition reimbursement.

Qualifikationen

• Licensed LPN in Minnesota with no restrictions.
• Minimum 1 year healthcare experience; acute hospital or home care preferred.
• Strong interpersonal and teaching skills.
• Ability to work independently; strong organizational skills.
• Excellent written and oral communication; valid driver’s license.
• Access to reliable vehicle or public transportation.
• U.S. Citizenship or valid Alien Work Permit.
• Background checks and screenings required.

Aufgaben

• Plan, provide and evaluate nursing care per policy; consult Case Manager on changes.
• Assess symptoms and institute measures; report changes to Case Manager.
• Document observations and teaching clearly and concisely.
• Administer medications and treatments as ordered by physician and Case Manager.
• Initiate rehabilitative nursing procedure techniques as appropriate.
• Use professional judgement to adapt nursing measures to needs.
• Communicate with the interdisciplinary team.
• Maintain accurate clinical records and progress notes.
• Assist with scheduling medical appointments and community resources.
• Maintain safe home environments and report abuse or neglect.
• Participate in on-call process to ensure client care.
