Adara Home Health is seeking a Licensed Practical Nurse (LPN) in Minnesota to provide comprehensive 1:1 client care in home settings. The role reports to a Clinical Supervisor and requires travel to clients’ homes, with a typical Mon-Fri schedule and on-call rotations.

Restricted to Minnesota residents with an active LPN license; the position offers a range of benefits and a base salary of $60,000–$72,000 per year, depending on experience, location, and qualifications.

Qualifikationen

• Currently licensed as an L.P.N. in the state of Minnesota.
• At least 1 year of health care experience preferred (acute hospital or home care).
• Ability to make appropriate nursing judgments.
• Strong interpersonal and teaching skills.
• Strong organizational skills and ability to work independently.
• Excellent written and oral communication skills.
• Proof of negative Mantoux or negative chest X-ray documentation.
• CPR certification is highly recommended.
• Access to dependable vehicle or public transportation for travel to multiple client sites daily.
• Valid driver's license and car insurance when using a personal vehicle for company business.
• U.S. Citizenship or valid Alien Work Permit evidence.
• Disclose any criminal history related to health services; past convictions may affect employment.
• Completion of background checks including state and federal screenings.

Aufgaben

• Plans, provides, and evaluates nursing care per established policies; consults Case Manager as needed.
• Recognizes symptoms, reports significant changes per Minnesota LPN scope.
• Documents observations and teachings clearly and completely.
• Administers medications and treatments as ordered by physician; delegated by Case Manager.
• Initiates rehabilitative nursing procedures as appropriate.
• Uses professional judgment to adapt nursing measures to changing needs.
• Communicates effectively with the interdisciplinary team.
• Participates in client conferences; maintains timely and accurate records.
• Assists clients in arranging medical appointments and connecting with community resources.
• Maintains safe client environment and reports accidents or suspected abuse or exploitation.
• Participates in ongoing professional development and education; adheres to confidentiality.
• Promotes positive client and employee service image; adapts to family diversity and home environment.
• Performs other duties as assigned and participates in after-hours on-call process.
