Job summary

Cherish-Hospice seeks an on-the-road CNA Hospice Aide to support patients in Kokomo and surrounding counties. You will provide direct personal care and related services to hospice patients under a plan of care.

Travel up to 50% is expected, with CNA and driver’s licenses required. Hospice and geriatrics experience are preferred, enabling compassionate, quality end-of-life care on the road.

Qualifications

• CNA license required.
• Driver's license required.
• Hospice experience preferred.
• Geriatrics experience preferred.
• Hospice and Palliative Medicine is a plus.

Responsibilities

• Provide personal care according to the patient’s needs, increasing comfort and maintaining hygiene in a safe environment.
• Care provided is in accordance with an established plan of care and under the supervision of a RN.
• Observe the patient, report observations and document care performed.
• Document the care provided and patient response in accordance with policies and regulatory standards.
• Provide timely documentation of patient services based on plan of care.
• Adhere to company policies and regulatory requirements and directives.
