Job summary

Servant's Heart Home Health Services, Inc. seeks a dedicated Home Health Aide (HHA) or CNA to provide compassionate, patient-centered care in clients’ homes in Kokomo, IN.

You will support ADLs, mobility, and daily living activities under supervision of a Registered Nurse, promoting independence and dignity. Responsibilities include executing care plans, documenting visits per HIPAA standards, coordinating with the care team, and traveling to client homes within the service area.

Qualifications

• Current HHA certification as required or per state rules
• Experience in home care or senior care preferred
• 6–12 months caregiving experience preferred
• Compassionate, dependable, service-oriented mindset
• Ability to follow care plans and work independently in a home setting
• Valid driver's license, reliable transportation, and current auto insurance
• Willingness to travel within the service area, including some extended drive times
• CPR certification preferred

Responsibilities

• Assist clients with activities of daily living (ADLs) such as bathing, grooming, toileting, dressing, mobility, and hygiene
• Provide light housekeeping, meal preparation, and companionship to reduce isolation
• Follow the established care plan and report changes to the supervising RN
• Communicate with clients, families, and care team; participate in care coordination
• Complete accurate visit documentation per agency HIPAA and safety standards
• Travel to client homes within the assigned service area and manage schedule to ensure reliable care
