Job summary

Home Sweet Home Healthcare in Arizona is seeking compassionate Registered Nurses (RNs) to join our growing Home Health team serving Maricopa County. You will provide skilled nursing care in patients' homes, including assessments, wound care, medications, and care plan updates, across Mesa, Gilbert, Chandler, and Queen Creek.

This PRN/part-time role offers flexible scheduling, autonomy in the field, and meaningful one-on-one patient relationships.

Qualifications

• Current Arizona RN license in good standing.
• Graduate of an accredited nursing program (BSN preferred).
• Minimum of 1 year of RN experience.
• Home Health nursing experience strongly preferred.
• Experience working with geriatric patient populations preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and organizational abilities.
• Valid driver’s license, reliable transportation, and ability to travel throughout the East and South East Valley area.

Responsibilities

• Conduct comprehensive nursing assessments in patients’ homes.
• Deliver skilled nursing care including wound care, medication management, treatments, and patient monitoring.
• Develop, implement, and update individualized home health care plans.
• Educate patients and caregivers on disease management, medications, safety, and ongoing care needs.
• Collaborate with physicians, case managers, therapists, and interdisciplinary care teams.
• Complete accurate and timely clinical documentation in compliance with agency and regulatory standards.
• Monitor patient progress and communicate changes in condition to the care team.
• Support improved patient outcomes, independence, and quality of life.
