Job summary

Home Sweet Home Health seeks an experienced Registered Nurse to provide skilled care in patients' homes across the East and South East Valley, including Mesa, Gilbert, Chandler and Queen Creek.

The role offers a Mon-Fri 8:00 AM–5:00 PM schedule with autonomy and the chance to build meaningful, one-on-one patient relationships while traveling throughout Maricopa County.

Qualifications

• Current Arizona RN license in good standing.
• Graduate of an accredited nursing program (BSN preferred).
• Minimum of 1 year RN experience; home health experience preferred.
• Experience with geriatric patients preferred.
• Strong clinical assessment and critical thinking skills.
• Valid driver’s license and reliable transportation; travel throughout East and South East Valley.
• Ability to work independently while delivering mobile home health care.

Responsibilities

• Conduct comprehensive nursing assessments in patients’ homes.
• Deliver skilled nursing care including wound care, medication management, treatments, and patient monitoring.
• Develop, implement, and update individualized home health care plans.
• Educate patients and caregivers on disease management, medications, safety, and ongoing care needs.
• Collaborate with physicians, case managers, therapists, and interdisciplinary care teams.
• Complete accurate and timely clinical documentation in compliance with agency and regulatory standards.
• Monitor patient progress and communicate changes in condition to the care team.
• Support improved patient outcomes, independence, and quality of life.

Home Health RN Responsibilities

As a Home Health Registered Nurse (RN), you will provide skilled nursing services throughout Maricopa County and surrounding Arizona communities. Responsibilities include:
