Job summary

Amcare Pro Home Health is seeking a Home Health RN Case Manager for PRN/full-time field positions covering Arlington, Fort Worth, Mansfield, Richland Hills and surrounding areas. You will perform SOC, ROC, and discharge visits and conduct OASIS assessments.

The role requires Texas RN licensure, 2+ years of nursing experience in home health, and familiarity with EMR systems (KINNSER/AXXESS). Valid CPR and reliable transportation are required.

Qualifications

• Must currently be licensed in Texas
• At least 2 years nursing experience
• IV infusion experience is a plus
• Prior home health care experience is a must
• Reliable transportation & car insurance
• Pass background check & have CPR
• Experience with Oasis E and Wellsky/Kinnser is a plus
• Knowledge of ICD-10 codes is a plus

Responsibilities

• Conduct SOC, ROC, recertification, and discharge visits
• Complete OASIS assessments accurately and timely
• Develop and manage individualized Plans of Care
• Provide skilled nursing care (wound care, med management, disease management, IV therapy as needed)
• Coordinate care with physicians and interdisciplinary team
• Educate patients and caregivers on treatment plans and safety
• Ensure compliance with Medicare and Texas home health regulations
• Complete all documentation in the agency EMR within required timeframes
