Lifespan Therapy is seeking a Home Health Physical Therapist to deliver in-home geriatric care across California. You’ll set your schedule and manage your caseload through our app-based system, enjoying autonomy and flexible hours.

Provide skilled PT evaluations, develop plans of care, and implement interventions focused on mobility, balance, and fall prevention. You’ll document efficiently using TherapySync and collaborate with a caring care team to optimize patient outcomes.

Qualifications

• Valid California Physical Therapist license.
• CPR or BLS certification.
• Driver's license and auto insurance.
• Physical + TB test within the last year.
• Professional liability and business license if applicable.

Responsibilities

• Provide Skilled PT care with in-home evaluations and individualized plans of care.
• Implement interventions focusing on functional strengthening, gait, balance, and fall prevention.
• Manage your caseload and schedule home visits based on availability and patient needs.
