Job summary

Lifespan Therapy is seeking a Home Health Physical Therapist to provide patient-centered care to geriatric homebound clients across California. Join our app-based network that emphasizes autonomy, flexibility, and independence.

In this role you will perform in-home evaluations, develop individualized plans of care, and deliver interventions to improve mobility, balance, and fall prevention. California license, CPR or BLS, driver’s license and TB testing are required.

Qualifications

• Valid California Physical Therapist license.
• CPR or BLS certification.
• Driver's license and auto insurance.
• Physical + TB test (within the last year).
• Professional liability and business license (if applicable).

Responsibilities

• Conduct in-home evaluations and develop individualized plans of care for geriatric patients.
• Implement functional strengthening, gait training, balance, and fall risk reduction interventions.
• Schedule and conduct home visits based on availability and patient needs.
• Collaborate with a care team to ensure coordinated patient care.
