Job summary

Remedy Home Health Care is seeking a licensed Physical Therapist (PT) in Ventura County for home health services across Ventura County, including Oxnard, Valencia, and surrounding areas. The role offers Per Diem/PRN, part-time, or full-time opportunities with autonomy and direct patient impact.

You will deliver skilled rehab in home-based settings, focusing on geriatrics, mobility, and independence, while collaborating with nurses, case managers, and physicians to optimize patient outcomes.

Qualifications

• Licensed Physical Therapist in California is required.
• CPR certification (AHA) is required.
• Minimum 1 year of clinical experience; home health preferred.
• Experience with geriatrics or senior care is preferred.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Conduct in-home physical therapy evaluations and reassessments.
• Develop and implement personalized rehabilitation care plans.
• Improve strength, balance, mobility, and functional outcomes.
• Provide therapy across home health, assisted living, and community settings.
• Educate patients and families on fall prevention and safe mobility.
• Document visits using WellSky EMR and collaborate with teams.
