Job summary

San Francisco Community Health Center (SFCHC) is hiring a Licensed Vocational Nurse (LVN) / Licensed Psych Tech (LPT) to provide mobile nursing care and case management for HIV-positive clients in Intensive Case Management and Enhanced Care Management programs.

The role involves field and office work, medication support, and collaboration with a mobile team to update care plans and ensure adherence and health education. SFCHC emphasizes equity and community health across diverse populations.

Qualifications

• Must hold a California LVN/LPN license.
• Experience with HIV/AIDS or LGBTQ populations is preferred.
• Strong communication in English, both speaking and writing.

Responsibilities

• Provide mobile nursing care and case management to HIV positive clients in ICM/ECM.
• Support field-based health and psychiatric assessments and monitoring.
• Dispense medications and support DOT/injections as appropriate.
• Coordinate with pharmacies and primary care providers weekly.
• Document in Epic and maintain treatment plans timely.
