Job summary

Responsibilities

• Provide direct nursing care to assigned patients within the LVN scope and ensure safe, patient-centered care.
• Assist with client assessments, care plan implementation, treatment adherence education, and monitoring to support HIV care engagement.
• Coordinate referrals to medical, behavioral health, and supportive services; collaborate with the multidisciplinary team to address barriers to care.
• Maintain accurate clinical documentation and support quality improvement activities in line with Ryan White Program standards.
• Engage in outreach and retention activities to improve access to care and health outcomes for program participants.
