Job summary

Deborah Heart and Lung Center in New Jersey seeks a Community Outreach Team Member – Clinical to support RuralCare Connect. You will participate in mobile health screenings, health education, and care navigation across Burlington and Ocean Counties, coordinating with clinic teams.

This grant-funded, full-time role requires flexibility for daytime, evening, and weekend events and travel throughout the service area.

Qualifications

• Must be flexible to support daytime, evening, and weekend hours.
• Travel throughout the service area is required.
• Associates degree or higher required or preferred.

Responsibilities

• Perform basic screening services (blood pressure, glucose, cholesterol) and other approved preventive screenings.
• Support patient engagement, referrals, and follow-up coordination with RuralCare Connect call center and clinical teams.
• Participate in mobile health screenings and outreach events as part of the RuralCare Connect program.
