Job summary

Deborah Heart and Lung Center is seeking a Registered Nurse (RN) Navigator to deliver coordinated, patient-centered care within a multidisciplinary team. The RN Navigator coordinates a plan of care across the system and serves as the single point of contact for referring physicians, patients, and caregivers.

Required: NJ State RN license and 1-3 years clinical experience; BLS. Preferred: Cardiac/critical care or ambulatory experience.

Qualifications

• 1-3 years clinical experience required.
• Experience in cardiac, critical care, or ambulatory settings preferred.
• NJ RN license and BLS certification required.
• Excellent interpersonal and communication skills; ability to work in a fast-paced, complex environment.

Responsibilities

• Coordinate plan of care across multidisciplinary team.
• Serve as single point of contact for referring physicians, patients and caregivers.
• Provide resources and assistance in accessing clinical and supportive care services.
• Collaborate with physicians, nurse practitioners, medical assistants, behavioral health specialists and others to provide comprehensive care.
