Job summary

Zufall Health is seeking a Registered Nurse in New Jersey to join our ambulatory care team. The RN will collaborate with physicians, nurse practitioners, and support staff to deliver patient-centered, cost-effective care in clinic, mobile health van, and community settings.

Responsibilities include triage, education, vaccination coordination, and liaison work with NJIIS/VFC, with emphasis on quality outcomes and family-centered care. Strong communication and organizational skills are essential.

Qualifications

• Current RN license in New Jersey.
• Current BLS certification.
• Minimum of 1 year experience in ambulatory care/community health setting.
• Experience with EMR and care coordination.
• Bilingual Spanish/English preferred.

Responsibilities

• Collaborate with Care Team to coordinate patient care and ensure high-quality outcomes.
• Perform triage in clinic or by phone; document interventions in EMR.
• Develop and implement policies/procedures with site leadership.
• Oversee laboratory services, including point-of-care testing and HIV testing.
• Serve as NJIIS/VFC liaison and organize staff trainings as needed.
• Educate patients on tests, procedures, vaccines, and preventive care.
• Coordinate patient appointments with providers and subspecialists.
• Maintain confidential records and participate in quality improvement.
