Qualifications

• Completion of certified APP program (Nurse Practitioner or Physician Assistant) with a master's degree.
• Minimum of one year clinical experience.
• Maryland license as Nurse Practitioner or Physician Assistant; CPR/BLS; valid driver's license.
• Leadership, clinical management, program development, infection control and quality improvement.

Responsibilities

• Provides quality primary healthcare and mobile health services to targeted populations.
• Collaborates with primary care providers and other agencies to develop care plans.
• Develops individualized care plans with patient, family and care team.
• Precepts and mentors treatment team members and supports professional development.
• Promotes health education and community screening initiatives.
• Coordinates care with physicians and specialists to ensure seamless services.
