Job summary

Brigade Health is seeking a compassionate Nurse Practitioner or Physician Assistant to join our Mobile Primary Care Team, delivering medical care to older adults where they live. This role emphasizes autonomy with support from multidisciplinary experts and a patient-centered approach.

Based in California, you will conduct assessments, develop care plans, and manage chronic conditions across homes and senior living settings.

Qualifications

• Active, unencumbered NP or PA license in the state you will be practicing.
• Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred).
• Compassionate, patient-centered approach and strong clinical judgment.
• Valid driver’s license and reliable transportation.

Responsibilities

• Deliver personalized, high-quality care to older adults in homes and senior living settings.
• Conduct comprehensive assessments and develop individualized care plans.
• Manage chronic conditions with skill and empathy.
• Collaborate across clinical disciplines (primary, psych, palliative, neuro) for whole-person care.
• Build trusted relationships with residents, caregivers, and community partners.
