Job summary

Brigade Health is seeking a compassionate Mobile Primary Care Nurse Practitioner or Physician Assistant to deliver care to older adults in homes and senior living communities. The role emphasizes autonomy with support, collaboration across disciplines, and flexibility to meet patient needs.

You will manage chronic conditions, perform comprehensive assessments, and build strong relationships with residents and caregivers while ensuring thorough documentation and care coordination.

Qualifications

• Active, unencumbered NP or PA license in the state you will be practicing.
• Minimum of 1–2 years of clinical experience (geriatrics or primary care preferred).
• Compassionate, patient-centered approach and strong clinical judgment.
• Valid driver’s license and reliable transportation.

Responsibilities
