Qualificações

• Active NP or PA license.
• 1+ years of primary care or geriatrics experience preferred.
• Strong communication and clinical skills.
• Valid driver’s license and reliable transportation.

Responsabilidades

• Provide mobile primary care visits to seniors.
• Manage chronic and acute conditions.
• Perform comprehensive assessments & care planning.
• Collaborate with caregivers, facilities, and interdisciplinary teams.
• Deliver compassionate, patient-centered care.
• Maintain accurate EMR documentation.
