Job summary

Florida Internal Medicine Associates is seeking a motivated and experienced Nurse Practitioner to provide on-site medical care to patients in long-term care and residential facilities. This mobile role offers autonomy, relationship-building, and meaningful impact on the elderly population.

The position serves ALFs, ILFs, and SNFs across Broward and Palm Beach counties with flexible scheduling and strong administrative support, offering opportunities for growth in a rapidly expanding practice.

Qualifications

• Active Florida ARNP license.
• Board Certification (ANCC/AANP) required.
• Medicare credentialed a plus.
• Experience in geriatrics, internal medicine, or long-term care preferred.
• Strong clinical assessment and diagnostic skills.
• Ability to work independently and manage a flexible schedule.
• Reliable transportation; travel between facilities required.

Responsibilities

• Provide primary and preventive care to geriatric patients in ALF/ILF/SNF settings.
• Perform comprehensive assessments, routine follow-ups, and acute visits.
• Manage chronic conditions (HTN, diabetes, dementia, etc.).
• Coordinate care with facility staff, specialists, and families.
• Complete documentation in EMR in a timely manner.
• Rotating weekend call.
• Participate in care planning and interdisciplinary team meetings.
• Ensure compliance with regulatory and clinical guidelines.
