Job summary

Florida Internal Medicine Associates is seeking a Geriatric Nurse Practitioner (ARNP) for a mobile facility-based care role across long-term care settings in South Florida. The provider will deliver primary and preventive geriatric care with autonomy and strong collaboration with facility staff.

Ideal candidates hold an active Florida ARNP license, ANCC/AANP board certification, and Medicare credentialing is a plus.

Qualifications

Active Florida ARNP license.
Board-certified in ANCC/AANP.
Medicare credentialed a plus.
Experience in geriatrics, internal medicine, or long-term care preferred.
Strong clinical assessment and diagnostic skills.
Ability to work independently and manage a flexible schedule.
Reliable transportation; travel between facilities required.

Responsibilities

Provide primary and preventative care to geriatric patients in ALF, ILF, and SNF settings (mobile).
Perform comprehensive assessments, routine follow-ups, and acute visits.
Manage chronic conditions (HTN, diabetes, dementia, etc.).
Coordinate care with facility staff, specialists, and families.
Complete documentation in EMR in a timely manner.
Rotating weekend call.
Participate in care planning and interdisciplinary team meetings.
Ensure compliance with all regulatory and clinical guidelines.
