Job summary

Lincare is seeking a dedicated care professional to perform patient follow-up and assess adherence to physician orders in home health settings. You will educate patients, conduct thorough assessments, and ensure insurance coverage for services, while maintaining detailed records and coordinating supply delivery to patients’ homes.

The role requires strong interpersonal skills, knowledge of home health practices, and ongoing professional development to support high-quality patient care.

Qualifications

• One year experience in home health care or related setting preferred.
• Experience in Dietetics, Nutrition, Home Health, or Nursing preferred.
• Excellent human relations skills to interface with doctors, referral sources, and patients.
• LPN/DTR/CNA experience preferred.

Responsibilities

• Educate patients as part of care activities and policy.
• Perform comprehensive patient assessments per procedures.
• Maintain knowledge of enteral home care techniques.
• Represent the company through professional appearance and reports.
• Coordinate information from referrals to ensure insurance coverage.
• Ensure patient records meet accreditation standards.
• Deliver and pick up enteral supplies to patients’ homes.
• Provide continuous patient care and coordinate services.
• Promote safe care environments and proper equipment use.
• Engage in ongoing education through training and in-services.
