Job summary

Carle Health is seeking a Hospital Social Worker to provide psychosocial assessments, counseling, and trauma-informed care in the Emergency Department, with field responses into the community to deliver mobile mental health services in the C-U metro area.

You will collaborate with EMS, ED staff, and community resources to facilitate timely discharge, coordinate support services, and address barriers to care, including advance directives and conservatorship where appropriate.

Qualifications

• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in Illinois.

Responsibilities

• Provides psychosocial assessments, supportive counseling, and crisis intervention.
• Field response into the community with EMS to offer mobile mental and behavioral health services.
• Coordinate with ED staff and security for next of kin notifications and mandated reports.
• Connect patients with community resources and transportation for medical appointments.
