The role includes field response several times daily and supervision of licensed social workers.

The candidate will perform follow-up case management to connect individuals to ongoing services, while ensuring HIPAA/compliance and professional standards

Qualifications

• Licensed Social Worker (LSW) in Illinois required.
• Driver's license and proof of auto insurance.
• At least 3 years of related experience preferred.

Responsibilities

• Actively listen to presenting issues and use crisis interventions to ensure safety.
• Provide mobile crisis services in community settings.
• Supervise Licensed Social Workers and Licensed Clinical Social Workers.
• Maintain timely documentation in medical records and required reporting.
