Job summary

Mentor Community Services, part of Sevita, seeks a Direct Support Professional Mobile to support individuals with developmental disabilities in community settings, working with clinical staff on plans.

Responsibilities include transportation, daily living assistance, skill-building, and behavior support to help individuals live as independently as possible. A compassionate, reliable candidate with a valid license is needed.

Qualifications

• High School Diploma or GED is required/preferred.
• Must be minimum of 18 years of age.
• Six months of experience in human services preferred.
• Valid driver’s license in good standing.
• Access to a registered vehicle with proof of insurance.
• Background checks cleared.
• Compassionate, reliable and committed to quality care.

Responsibilities

• Transportation and support out in the community: outings/field trips, medical appointments, and shopping.
• Assist with daily living skills including meal preparation and medication administration.
• Guide on developmental goals: community participation, budgeting, exercise, nutrition, vocational tasks.
• Support living skills: personal hygiene, grooming, bathing, toileting.
• Help redirect agitated or aggressive behaviors.

Skills

Six months of experience in human

Valid driver’s license
