Requires a Master’s Degree in mental health and licensure eligibility (LCSW/LMFT/LPC). The position focuses on mobile crisis services, evidence-based practice, and community-based care for children and families.

Qualifications

• Master’s Degree or higher in mental health field.
• LCSW, LMFT, LPC eligible; licensed preferred.

Responsabilités

• Provides assessment and treatment interventions with fidelity to the applicable model.
• Documents ongoing client progress and outcomes.
• Demonstrates competency in age-related or specialty issues for each population served.
• Develops discharge plans utilizing natural supports with clients.
• Maintains collaborative relationships with family and other providers.
• Provides crisis assessment and intervention to monitor crisis status.
• Performs home and community-based care.
• Maintains client and staff records per agency policy.
• Provides coverage for team members during absences.
• Participates in weekly supervision and meetings to review progress.
• Transports clients using agency or personal vehicles when necessary.
• Performs additional duties as required.
