Wellstone is seeking a part-time Adult Mobile Crisis Team Case Manager for weekends in Cullman County. The role focuses on connecting clients to resources, coordinating care, and supporting transition from crisis to recovery.

Qualifications

  • Bachelor’s degree in Human Services, Psychology, Counseling or social sciences field.
  • Acceptable 5 year driving record & reliable transportation.
  • Excellent verbal and written communication skills.
  • Knowledge of local community to include agencies and resources.

Responsibilities

  • Provide outpatient case management services to the seriously mentally ill population.
  • Provide direct services to severely mentally ill clients to address needs which are unmet as well as information about continuum of care.
  • Monitor medication compliance, maintain awareness of side effects, and ensure client is taking medication as directed.
  • Maintain high quality client documentation and records management utilizing electronic charting.
  • Develop and implement Individual Service Plan with goals, resources and interventions.
  • Review client’s progress regularly and revise plans as needed.

Job Description: Adult Mobile Crisis Team Case Managers work closely with our Cullman County First Responders, First Responder Teams, Crisis Teams and Therapeutic Teams in a partnership to ensure we provide every appropriate resource and support needed to our eligible patrons as they transition from crisis to care and from care to recovery. This position requires organization and communications skills, the ability to connect and evaluate situations quickly, and knowledge of our local providers. As the Case Manager, you are the link and liaison for your client. There is one part-time position available working Saturdays and Sundays, 7:00PM to 7:00AM.What you’ll be doing:Provide outpatient case management services to the seriously mentally ill population.Provide direct services to severely mentally ill clients to address needs which are unmet as well as information about continuum of care;Provide training on money management as it relates to meeting needs of the client to obtain and maintain psychiatric stability residential setting;Provide follow-up progress as per need of client to treatment team members.Evaluate client’s residential stability and assist those clients who are in need of more stable environment.Provide Basic Living Skills training when necessary as it relates to clients immediate needs while preparing client for less intensive level of care within the community;Assist clients with obtaining medication.Monitor medication compliance, maintain awareness of common side effects, and ensure client is taking medication as directed on a regular basis.Provide traditional case management services as well as collateral services such as transportation to appointments, referral agencies, etc.Maintain high quality client documentation and records management utilization electronic charting.Monitor the care and services clients receive in residential setting.Develop a positive working relationship with Residential Care manager/owner to meet needs of clients.Develop and implement Individual Service Plan which consist of goals, resources, and interventions.Review client’s progress every three months and revise Individual Service Plan as needed.Review SUN-R 6 months after initial assessment and then at intervals of 12 months thereafter.Revise client’s goals once met or progress as they occur.Assist mentally ill clients through crisis situations and/or arranging for the provision of such assistance by other professional or personal caregivers.Perform incidental care to families of the mentally ill and assist those family members in understanding the mental illness.Complete other duties as assigned.This job description is only a summary of the typical functions of the job and is not designed to be an exhaustive or comprehensive list of all possible duties, tasks, or responsibilities that are required of the employee as they may change, or new ones may be assigned at any time with or without notice.What we’re looking for:Bachelor’s degree in Human Services, Psychology, Counseling or social sciences fieldAcceptable 5 year driving record & reliable transportationExcellent verbal and written communication skillsSuccessful completion of State of Alabama Adult Case Management TrainingKnowledge of local community to include agencies and resourcesPersonal Automobile Liability Insurance minimum amounts allowed by stateWe are compassionate towards those impacted by behavioral health disorders.We are dedicated to one another through collaboration and teamwork.We are optimistic problem-solvers who do what it takes to get the job done.
