Job summary

AbsoluteCARE Medical Center & Pharmacy seeks a Community-Based Medical Assistant to support our CB-NP in homes and community settings. The role includes assisting with hospital follow-up visits, discharge planning, medication reconciliation, and patient education as part of our Integrated Care Team.

Travel to client homes and clinics is required. The candidate will manage scheduling, perform phlebotomy, assist with exams, and ensure smooth transitions of care for high-risk patients.

Qualifications

• Minimum: Certified/Registered Medical Assistant in state or valid EMS certification.
• 3 years of Medical Assistant experience for MA track.
• EMT candidates require at least 1 year pre-hospital experience.
• Active CPR certification is required.
• Proficiency with Microsoft Office applications is expected.

Responsibilities

• Provide community-based primary, transitional, and acute care support.
• Travel to member homes and community sites as needed.
• Manage CB-NP schedule and follow-up appointments.
• Prepare members for exams; take vitals and perform screenings.
• Close quality measure gaps and ensure chart completeness.
• Assist providers with exams, minor procedures, and phlebotomy.
• Review after-visit summaries and ensure patient understanding.
• Coordinate care tasks and maintain infection-control protocols.
