Job summary

Columbia River Mental Health Services is seeking a Case Manager for the Mobile Health Team serving individuals and families across Clark County. The role works with a multi-service treatment team to coordinate services in the clinic, home, and community.

You will educate clients about mental illness, help them achieve goals, and build skills to promote independence and recovery, while maintaining documentation and collaborating with providers.

Qualifications

• Bachelor’s degree in Mental Health, Social Work, Psychology, or Human Services preferred.
• Approximately 1–2 years of experience in the social service field with mental illness or chemical dependency preferred.

Responsibilities

• Develop a unique treatment plan with clients based on recovery goals.
• Coordinate care with other providers and community resources.
• Assist with crises and develop crisis plans ahead of need.
• Help clients access medical and dental appointments and services.
• Educate clients, families, and support networks about mental illness and treatment.
• Advocate for clients’ rights and ensure fair treatment.
