Job summary

Qualifications

• Demonstrated expertise in social work or psychology with evidence-based interventions.
• Strong case management skills to coordinate services and connect patients with community resources.
• Solid foundation in mental health assessment and treatment for diverse populations.
• Effective communication within interdisciplinary teams and with patients and families.
• Current licensure in North Carolina (LCSW/LCMHC/LMFT) or license-eligible.
• Experience in community health or integrated primary care, serving underserved populations preferred.
• Ability to travel between Charlotte-area clinic sites and adapt to mobile/float assignments.
• Proficiency in documentation and EHR adherence to regulatory/ethical standards.

Responsibilities

• Conduct psychosocial evaluations and develop person-centered treatment plans.
• Deliver individual and group counseling services.
• Coordinate care with medical providers and support staff.
• Provide case management and referrals to community resources.
• Assist with crisis intervention and follow-up to ensure continuity of care.
