Job summary

Qualifications

• Master’s degree required in Social Work or Counseling.
• Experience providing case management services.
• Experience with vulnerable patient populations.
• Experience with serious mental illness and/or substance use disorders.
• Knowledge of behavioral health and medical settings.
• Ability to work with diverse, safety-net population.
• Bilingual/multilingual preferred.

Responsibilities

• Identify and engage appropriate clients for complex care management.
• Perform comprehensive bio-psycho-social-spiritual assessments.
• Develop client-centered care plans with the care team.
• Refer clients to medical, behavioral health, housing, transportation, and financial services.
• Provide ongoing outreach and monitoring of care utilization.
• Collaborate with community partners and service providers.
• Participate in team meetings and clinical reviews.
• Address behavioral health crises and develop crisis plans.
• Maintain HIPAA compliance and accurate documentation.
• Utilize EMR for tracking, billing, and communication.
