Qualifications

Brentwood Hospital is looking for a dynamic Mobile Assessment Coordinator to join our team of professionals who are dedicated to providing help and hope to individuals and families for the achievement of improving their mental health. Brentwood has served Louisiana, East Texas and South Arkansas for more than 45 years. We provide quality services to children, adolescents, adults, and seniors. We help individuals and families achieve optimal mental health, resulting in improved quality of life. A dedicated professional team provides individualized treatment, meeting the personal needs of our patients in a safe and nurturing environment.

ESSENTIAL JOB FUNCTIONS:

  1. Participate in screening patients for admission into hospital programs, utilizing pre-approved admissions criteria.
  2. Provide assessments of incoming patient’s mental health/psychiatric condition for mentally ill, emotionally disturbed, through various methods.
  3. Prepare written reports of assessment results and prepares recommendations for the level of care and needs.
  4. Interface with the facility staff who will provide the appropriate level of care, to ensure a smooth transition for the patient.
  5. Promote an effective use of resources for patients, customers and the facility with sensitivity to cost of health care.

Qualifications

  • Education/Licensure: Licensed /Registered Nurse or Master’s Degree in social work, psychology, or mental health related field plus applicable licensure
  • Experience: A minimum of two (2) years direct clinical experience in a psychiatric or mental health setting, with direct experience in patient assessment, family motivation, treatment planning, and communication with external review organizations
  • Hours: Determined by the Director of Intake
