Job summary

Options for Southern Oregon is seeking an Addictions Counselor to join our mobile outreach team in Medford, OR. You will provide outreach and engagement for individuals with behavioral health needs, build trust, and facilitate entry into needed treatment in the community.

The role involves ASAM assessments, treatment planning, and coordinating with community partners. CADC-R registration is required or pursued, with CADC I/II preferred, and a schedule of Mon-Fri, 8am-5pm with driving between

Qualifications

• Experience with individuals who have substance abuse/addiction.
• Interest and ability to register as a CADC-R.
• Preferred: CADC I or CADC II (Certified Alcohol and Drug Counselor).
• Proficiency with email, document handling, and MS Office/EDR systems.

Responsibilities

• Conducts ASAM Assessments, completes treatment plans, and initiates services.
• Outreaches to clients and uses motivational interviewing to engage clients in needed services.
• Transports clients to facilitate client-directed services and travels between sites.
• Keeps up-to-date documentation and scheduling; uses the EHR system.
• Acts as liaison to community resources to enhance independent living skills.
