Job summary

Mentor Community Services, part of the Sevita family in the East Bay, seeks a Clinical Coordinator to oversee services, case management, and treatment implementations for individuals served.

You will lead quality care, ensure compliance with contracts and regulations, and travel throughout Alameda and Contra Costa Counties as needed. Join a mission-driven team dedicated to empowering lives and delivering compassionate supports.

Qualifications

• Bachelor’s degree in human services or related field.
• One year of related work with individuals with intellectual and developmental disabilities, brain injury, or youth at-risk preferred.
• Equivalent combination of education and experience accepted.
• Current driver’s license, car registration, and auto insurance.
• Ability to travel throughout Alameda and Contra Costa Counties.
• Licensure and training as required by state (e.g., CPR, First Aid, Behavioral Intervention Techniques).
• QMRP/QIDDP as required by state.
• A reliable, compassionate attitude and commitment to quality in everything you do.

Responsibilities

• Coordinate the development, review, and implementation of services, case management, behavior intervention plans, and treatments provided to individuals served.
• Oversee all services provided to individuals and their families, monitor quality of care, ensure contractual compliance, and monitor regulatory program compliance.
• Assist in developing quality services and engaging activities meeting individual needs.
• Build and maintain relationships with families and external case managers.
