Job summary

Mentor Community Services, part of Sevita, seeks a Clinical Coordinator for the East Bay to coordinate services, case management, and behavior plans. The role involves overseeing service delivery, ensuring quality and compliance, and building strong family relationships across Alameda and Contra Costa Counties.

This is a full-time position requiring travel and relevant licensure. The ideal candidate holds a BA in human services or related field, has some related experience, and demonstrates

Qualifications

• Bachelor’s degree in human services or related field.
• One year of work-related experience with individuals with IDD, brain injury, at-risk youth preferred.
• Equivalent combination of education and experience accepted.
• Current driver’s license, car registration and auto insurance.
• Ability to travel throughout Alameda and Contra Costa Counties.
• State-required licensure/training (CPR, First Aid, Behavioral Intervention Techniques).
• QMRP/QIDDP as required by state.
• Reliable, responsible attitude and compassionate approach.
• Commitment to quality in everything you do.

Responsibilities

Program Services Clinical Coordinator - East Bay

Compensation - $45,760 per year plus overtime
