The City of Rockford Public Service Departments: Fire Department seeks an experienced firefighter for an internal MIH program role in Rockford, IL. This position requires active duty status with the Rockford Fire Department and performs duties to support the Mobile Integrated Healthcare program.

Responsibilities include coordinating home visits with clients, assisting case management teams, documenting interactions, and ensuring HIPAA compliance.

Competenze

• Five years experience as a Rockford Firefighter or equivalent training.
• Possession of Illinois Class B driver’s license (non CDL).
• Certification as Firefighter III or Advanced Technician Firefighter.
• Certification as Instructor I or willingness to obtain within 12 months.
• Licensure as EMT-Paramedic (or PHRN).
• Licensure/certification as Community Paramedic or willingness within 12 months.

Mansioni

• Management of the Department's Mobile Integrated Healthcare (MIH) Program.
• Assist with providing services and resources to community members identified as needing help.
• Identify patients who will benefit from participation in the MIH Program.
• Coordinate at home visits with clients and provide transportation for case management teams if necessary.
• Conduct home safety assessments and provide recommendations or corrections.
• Maintain documentation of all interactions with MIH participants using forms/software.
• Maintain knowledge of infection control practices and HIPAA regulations.
• Assist in preparing monthly/quarterly/yearly MIH program reports.
• Report to the EMS Training Coordinator regarding MIH program operations.
• Support department goals and maintain attendance.
• Assist EMS Training Coordinator in training activities.
