New Braunfels Fire Department seeks a Mobile Integrated Health (MIH) Paramedic to advance community health through proactive patient care coordination and case management. The role aims to reduce non-emergency 9-1-1 utilization by connecting vulnerable residents with health and social resources.

You will perform in-person visits, assessments, and care planning, while collaborating with physicians, home health agencies, and community partners.

Aufgaben

• Provide comprehensive patient care coordination and case management within MIH, including triage and high-utilizer management.
• Conduct in-person visits, medical assessments, and care plan development.
• Research resources and curate referrals to reduce barriers to care.
• Coordinate care with physicians, home health agencies, and social service partners.
• Support opioid misuse prevention and harm reduction through patient education and naloxone access.
• Assist patients with navigating insurance and benefit applications.
• Maintain confidential patient records and ensure data integrity.
• Track program data and submit monthly reports to City Manager’s Office.
• Represent the MIH program at meetings and community events.
• Provide guidance to external organizations interested in MIH programs.
• Perform Child Passenger Safety Technician duties.
• Participate in meetings and maintain certifications.
