Job summary

The Information Center is seeking a strategic Director of Care Management to lead the MI Choice Medicaid Waiver Program and the Community Transitions Program in Michigan. You will set the direction, build partnerships, and develop a high-performing team dedicated to helping older adults and people with disabilities live in the community.

Qualifications

• Bachelor's degree in health and human services or related field.
• Five+ years in a management role in a social service setting, focused on long-term care or HCBS.
• Knowledge of Medicare/Medicaid, MI Choice Waiver and Community Transitions.
• Experience with person-centered planning and addressing barriers to community discharge.
• Proven ability to supervise and motivate staff, mediate conflict, and solve complex problems.
• Strong communication skills for clarity and conveying importance.
• Competence in Microsoft Office and comfort with digital tracking systems like COMPASS and CHAMPS.
• Commitment to confidentiality in line with HIPAA and agency policy.

Responsibilities

• Lead the department and oversee program operations to meet contractual, accreditation, and agency goals while ensuring MDHHS and federal compliance.
• Supervise and mentor the MI Choice Manager, Vendor Contracts and Compliance Manager, and Community Transitions Social Workers.
• Evaluate program operations against quality protocols and develop remediation plans for continuous improvement.
• Cultivate partnerships with other organizations to extend program reach and impact.
• Represent TIC in statewide and community meetings, events, and conferences and share insights with the team.
• Collaborate across departments and contribute to strategic decisions about program direction.
