MH Residential Coordinator – Baldwin Location

Program Overview:

The Residential Coordinator will work closely with the SA/MH Residential Program Manager to support new clients as they enter services. This role ensures that individuals’ behavioral health, physical health, and other identified needs are addressed through effective coordination with River Edge programs, primary care, and community partners. The Residential Coordinator monitor, evaluates, and record client progress according to measurable goals described in treatment and care plan.

Key Responsibilities:

• Oversee services and supports provided to individuals.
• Supervise the development and implementation of the Individual Service Plan (ISP) for all waiver services, including annual reviews and any ISP addendums.
• Conduct assessments to support the development of individualized service plans.
• Supervise high-intensity services that address health and safety risks.
• Maintain confidentiality of all records related to client treatment.

• Ability to communicate both orally and in writing and modeling professional behavior.
• Ability to provide helpful input in determining appropriate services for individuals.

Preferred Qualifications:

• Bachelor’s degree in a related field (e.g., psychology, sociology, or similar) from an accredited college or university.
• Minimum of one (1) year of supervisory experience.
• Ability to obtain and maintain CPR and First Aid certification; CPI and other required trainings (e.g., HRST, SIS, ISP as offered by DBHDD).
• Must sign and adhere to the Developmental Disabilities Professional (DDP) job description.
