• Demonstrated knowledge of Georgia DBHDD developmental disability services and applicable regulatory requirements.
• Experience in residential services, crisis services, behavioral health, developmental disability services, or another high-acuity human-services environment preferred.
• Previous supervisory or leadership experience preferred.

Position Summary

The Crisis Coordinator provides operational, clinical, and regulatory coordination for Crisis Continuum Homes serving individuals with intellectual and developmental disabilities (IDD), including individuals with complex behavioral, psychiatric, and medical needs.

This position provides oversight and coordination across four Community Residential Alternative (CRA) locations and is responsible for ensuring services are person-centered, safe, appropriately documented, and delivered in accordance with Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD), Healthcare Facility Regulation (HFR), Medicaid waiver, and organizational requirements.

The Crisis Coordinator works closely with individuals, families and guardians, direct care staff, case managers, nurses, behavioral specialists, clinical providers, and organizational leadership. The position also participates in an after-hours/on-call rotation and provides guidance during crisis situations.

Duties And Responsibilities

PROGRAM OVERSIGHT AND REGULATORY COMPLIANCE

• Regulatory Compliance: Align all four residential operations with Georgia DBHDD policies, Healthcare Facility Regulation (HFR) standards, and federal waiver requirements.
• Quality Assurance: Translate DDP requirements into measurable outcomes, including service-plan completion, assessment timelines, and incident management.
• Audit Readiness: Conduct routine chart audits, identify documentation deficiencies, implement corrective actions, and prepare programs for DBHDD quality reviews or state investigations.
• Authorization Monitoring: Verify that daily care matches authorized support levels and individual service plan (ISP) requirements.

PERSON-CENTERED PLANNING AND ASSESSMENT SUPERVISION

• ISP Coordination: Facilitate the development, annual review, and addenda formulation of person-centered ISPs reflecting individual goals, strengths, and medical or behavioral risks.
• Clinical Assessments: Supervise and conduct initial and ongoing comprehensive assessments to identify changing clinical, physical, and psychological needs.
• Interdisciplinary Collaboration: Convene and lead meetings with individuals, families, case managers, nurses, behavioral specialists, and external clinical providers.

MULTI-SITE CRA OPERATIONAL LEADERSHIP

• Program Management: Provide cohesive leadership across four Crisis Continuum locations utilizing Community Residential Alternative (CRA) models.
• On-Call Management: Develop, maintain, and participate in a structured 24/7 supervisor on-call rotation to provide rapid crisis triage and emergency operational guidance.
• Workforce Direction: Supervise direct care staff, evaluate performance, validate competencies, manage emergency shift coverage, and address staffing vacancies.
• Cross-Site Communication: Deliver clear, actionable updates between home staff, administrative leadership, and external stakeholders to eliminate operational silos.

BEHAVIORAL AND INTENSIVE ACUITY CARE MANAGEMENT

• PBS Fidelity: Coordinate and monitor the execution of Positive Behavior Support (PBS) plans and proactive verbal de-escalation across behavioral-focused homes.
• Data-Driven Analysis: Ensure accurate and timely behavioral data collection; analyze trends regarding trigger frequency, duration, and severity to optimize intervention outcomes.
• High-Acuity Medical Tracking: Oversee safety protocols, enhanced staffing ratios, and restrictive safeguard regulations within intensive-acuity homes.
• Rapid Clinical Triage: Coordinate emergency medical or psychiatric evaluations, manage hospital step-down transitions, and lead multidisciplinary post-crisis case reviews.

DOCUMENTATION, ADVOCACY, AND STAKEHOLDER COORDINATION

• Health Record Integrity: Enforce accurate progress notes and incident reports while strictly safeguarding Protected Health Information (PHI) under HIPAA regulations.
• System Reporting: Review, record, and elevate critical incidents through the state-mandated DBHDD reporting infrastructure.
• Individual Advocacy: Champion client rights, choices, dignity, and independent community living opportunities.
• Stakeholder Relations: Serve as the primary professional liaison for families, legal guardians, state case managers, and community healthcare entities.

ADMINISTRATIVE AND OPERATIONAL INFRASTRUCTURE

• Program Scaling: Assist leadership in developing workflows, specialized staffing grids, and clinical escalation matrices for newly opening continuum homes.
• Policy Maintenance: Update site-specific manuals to immediately reflect changing DBHDD operational or environmental expectations.
• Resource Oversight: Monitor site safety, clinical supply inventories, and environmental conditions to ensure clean, safe, and therapeutic living spaces.
