Job summary

Brown University Health is seeking a Referral Coordinator to manage and implement referrals for patients within the HMA network, ensuring confidentiality and alignment with policy.

The role collaborates with physicians, office staff, and administrators to optimize patient flow, assess referral processes, and monitor adherence to guidelines.

Qualifications

  • Three to five years of experience in a health care delivery system.
  • Advanced computer skills, including knowledge of spreadsheets.
  • Ability to work independently and establish effective relationships with physicians, nurse practitioners, physician assistants, office staff, and other health care providers.
  • Ability to identify risk-prone or problematic situations and know when to seek supervisor assistance.
  • Proficiency with computer use and daily use of a headset for telephonic communication as necessary.

Responsibilities

  • The RC maintains strict patient confidentiality.
  • The RC attends bi-weekly staff meetings.
  • The RC assesses existing referral processes to ensure minimum standards within the PCHI/HMA network are met for referral review.
  • The RC evaluates office staff ability to comply with referral management goals and determines if assistance is required to effect changes in referral patterns.
  • The RC interacts with HMA administrators, site managers, PCPs, and the Medical Director to ensure patient flow and office procedures are not negatively impacted by changes in referral management requirements.
  • The RC reviews with the PCP referrals that fail to meet established guidelines and, under PCP direction, explores alternatives to referrals outside of the recommended guidelines.
  • The RC implements processes to track referral activity under the direction of the department Director, centralized in one location with monthly staff meetings that include all team members.

Job description

Summary

Acquires and implements referrals for patients with upcoming visits in-house and outside HMA, in accordance with facility policies and procedures. Demonstrates ethical conduct and acts as a positive role model and team member. Participates in facility committees, meetings, in-services, and activities. Minimum Qualifications
  • Three to five years of experience in a health care delivery system.
  • Advanced computer skills, including knowledge of spreadsheets.
  • Ability to work independently and establish effective relationships with physicians, nurse practitioners, physician assistants, office staff, and other health care providers in physician's offices, community agencies, hospitals, and other health care facilities.
  • Ability to make autonomous decisions using sound judgment based on factual information and established industry guidelines.
  • Ability to identify risk-prone or problematic situations and know when to seek supervisor assistance.
  • Ability to remain organized with workflow amid interruptions and to stay flexible while maintaining professionalism with patients, families, hospital staff, and co-workers.
  • Proficiency with computer use and daily use of a headset for telephonic communication as necessary.
Working Conditions

The work environment is typical of an office setting. Extensive computer work, data entry, and verification of system data are necessary. Duties require periods of sitting, walking, standing, reaching, climbing, balancing, stooping, kneeling, crouching, crawling, talking, and hearing.
