Job summary

The Occupational Therapist (OT) provides skilled occupational therapy services to patients in their homes according to physician orders and individualized plans of care.

The OT evaluates patients, develops and implements treatment plans, promotes independent functioning, provides patient and caregiver education, coordinates care with interdisciplinary team members, and documents services in compliance with agency policies and regulatory requirements.

Qualifications

• Graduate of an occupational therapy curriculum accredited jointly by the AMA Council on Medical Education and the American Occupational Therapy Association.
• Eligible for the NBCOT National Registration Examination.
• Registration to practice as an Occupational Therapist in the practicing state, if required.
• CPR Certification Required.

Responsibilities

• Perform patient evaluations and reassessments.
• Develop, implement, and update individualized treatment plans.
• Provide therapeutic interventions to improve safety, function, and independence.
• Educate patients, families, and caregivers on therapy programs and adaptive techniques.

• Maintain accurate and timely clinical documentation.
• Participate in quality improvement initiatives and case conferences.
• Ensure compliance with all regulatory, ethical, and organizational standards.

Job Summary

The Occupational Therapist (OT) provides skilled occupational therapy services to patients in their homes according to physician orders and individualized plans of care. The OT evaluates patients, develops and implements treatment plans, promotes independent functioning, provides patient and caregiver education, coordinates care with interdisciplinary team members, and documents services in compliance with agency policies and regulatory requirements. The OT also supervises Certified Occupational Therapy Assistants (COTAs) and Home Health Aides (HHAs) as appropriate.

Key Responsibilities

• Perform patient evaluations and reassessments.
• Develop, implement, and update individualized treatment plans.
• Provide therapeutic interventions to improve safety, function, and independence.
• Educate patients, families, and caregivers on therapy programs and adaptive techniques.
• Assess home environments and recommend modifications and adaptive equipment.

Qualifications

• Graduate of an occupational therapy curriculum accredited jointly by the Council on Medical Education of the American Medical Association and the American Occupational Therapy Association.
• Eligible for the National Registration Examination of the National Board for Certification in Occupational Therapy (NBCOT).
• Registration by the NBCOT and registration to practice as an Occupational Therapist in state of practice if state has registration practices.
• CPR Certification Required.
• Must be able to function in an environment with minimal direct supervision, accepting personal responsibility for maintaining professional relationships with the agency’s clients and their families
• Must accept responsibility for maintaining clinical practice skills, learning and adhering to agency’s policies and procedures on an on-going basis.
• Must have a minimum of one (1) year experience or have a preceptor for a minimum of 6-8 weeks.
• Home healthcare experience preferred.
• Must have current Driver’s license and reliable transportation to and from work site.
• Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics and adhere to the Compliance Program.

• Lifts, stands, and walks 35% of time.
• Bends, stoops, and twists 35% of time.
• Positions and lifts patients 10% of time.
• Ability to perform continuously detailed tasks with frequent interruptions and work reprioritizations (90% of time).
• Must be able to adapt to frequently changing work parameters
• Must be able to travel to various TH/THHS worksites or various work locations as required
• Must be able to lift 35 pounds
