Job summary

Trinity Health Mid-Atlantic is seeking a Physical Therapist for home health services, delivering skilled PT in clients’ homes under physician orders. You will assess, plan, and implement individualized treatment, coordinating with a multidisciplinary team and ensuring detailed documentation.

The role requires state licensure, APTA-approved training, and independence with minimal supervision. A valid driver’s license is required, with travel across Trinity Health locations and client homes.

Qualifications

• Graduate of an APTA-approved Physical Therapy program.
• Licensed Physical Therapist in the state of practice.
• Minimum one (1) year of experience or completion of a 6-8 week preceptorship.
• Home health experience preferred.
• Ability to work independently with minimal supervision.
• Commitment to maintaining clinical competence, organizational policies, and ethical standards.
• Valid driver’s license and reliable transportation required.
• Physical & Mental Requirements: lift up to 35 lbs; frequent standing, walking, bending, positioning; adaptable to schedules and environments; travel to client homes and Trinity Health locations.

Responsibilities

• Provide professional physical therapy services to home health clients per physician orders and plan of care.
• Develop and implement individualized treatment plans.
• Coordinate care with physicians and interdisciplinary team; communicate changes in patient condition.
• Ensure accurate and timely clinical documentation.

The Physical Therapist provides professional physical therapy services to home health clients in accordance with physician orders, the established plan of care, applicable state practice acts, licensure requirements, and organizational policies. This role performs comprehensive assessments, develops and implements individualized treatment plans, and delivers preventive, therapeutic, and rehabilitative interventions to promote functional independence and optimal outcomes in the home setting.

The Physical Therapist coordinates care with physicians and the interdisciplinary team, communicates changes in patient condition, and ensures accurate and timely clinical documentation. Responsibilities include patient and caregiver education, environmental assessments, discharge planning, quality improvement participation, and supervision of PTAs and Home Health Aides as permitted by state practice acts. The role requires independent clinical judgment, adherence to regulatory and ethical standards, and a commitment to quality, compliance, and patient-centered care.
