Job summary

MercyOne Home Care in the Mason City area is seeking a dedicated Occupational Therapist (Home Health) to provide skilled OT services in patients' homes under physician orders and individualized plans of care.

The OT will evaluate patients, develop treatment plans, promote independence, educate patients and caregivers, coordinate with an interdisciplinary team, document services, and supervise COTAs and HHAs as appropriate. A valid OT license and CPR certification are required.

Qualifications

• Graduate of an occupational therapy curriculum accredited jointly by the CMA and AOTA.
• Eligible to sit for NBCOT National Registration Examination.
• Registration by NBCOT and state licensure to practice OT where required.
• CPR certification required.
• Ability to work with minimal direct supervision and maintain professional relationships with clients and families.
• Minimum of one year of experience or completion of a 6–8 week preceptor.
• Home healthcare experience preferred.
• Must have a valid driver’s license and reliable transportation.

Responsibilities

• Participate in quality improvement initiatives and case conferences.
• Ensure compliance with regulatory, ethical and organizational standards.

The OT evaluates patients, develops and implements treatment plans, promotes independent functioning, provides patient and caregiver education, coordinates care with interdisciplinary team members, and documents services in compliance with agency policies and regulatory requirements. The OT also supervises Certified Occupational Therapy Assistants (COTAs) and Home Health Aides (HHAs) as appropriate.

Key Responsibilities

Qualifications

• Graduate of an occupational therapy curriculum accredited jointly by the Council on Medical Education ofthe American Medical Association and the American Occupational Therapy Association.
• Eligible for the National Registration Examination of the National Board for Certification in OccupationalTherapy (NBCOT).
• Registration by the NBCOT and registration to practice as an Occupational Therapist in state of practice ifstate has registration practices.
• CPR Certification Required.
• Must be able to function in an environment with minimal direct supervision, accepting personal responsibility for maintaining professional relationships with the agency’s clients and their families
• Must accept responsibility for maintaining clinical practice skills, learning and adhering to agency’s policies and procedures on an on-going basis.
• Must have a minimum of one (1) year experience or have a preceptor for a minimum of 6-8 weeks.
• Home healthcare experience preferred.
• Must have current Driver’s license and reliable transportation to and from work site.
• Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics andadhere to the Compliance Program.

Physical Requirements

• Lifts, stands, and walks 35% of time.
• Bends, stoops, and twists 35% of time.
• Positions and lifts patients 10% of time.
• Ability to perform continuously detailed tasks with frequent interruptions and work reprioritizations (90% of time).
• Must be able to adapt to frequently changing work parameters
• Must be able to travel to various TH/THHS worksites or various work locations as required
• Must be able to lift 35 pounds
