Job summary

Jamestown-S in Washington is seeking a Licensed Mental Health Therapist (LMHT) to support medically complex patients in outpatient care. The role emphasizes a whole-person approach, collaborating with primary and specialty care teams, including behavioral health and MAT, to improve quality of life.

The LMHT will conduct biopsychosocial assessments, provide direct therapy, utilize CBT and trauma-informed care, and participate in program improvement initiatives while maintaining timely

Qualifications

• Master’s degree in social work and licensure at LICSW, LMPHP or LMFT required.
• Three years post-licensure experience preferred.
• Experience in group facilitation and collaborative care planning.
• Knowledge of counseling techniques and crisis intervention.
• Must maintain WA state professional license.

Responsibilities

• Provide age and culturally appropriate comprehensive bio-psychosocial assessments for mental health, substance use, and medical needs in the primary care setting.
• Provide direct services to patients including screening, assessment, brief intervention, and appropriate referrals.
• Utilize evidence-based practices (CBT, motivational interviewing, trauma-informed care) to support patient care.
• Provide education and training to patients, families, and staff on behavioral health issues.
• Provide liaison services, referrals to hospital resources and community agencies including medication assistance programs.
• Collaborate with BH, MAT, and Chronic Disease Management to develop and facilitate group visits; participate in Quality Improvement activities.
• Complete patient behavioral records, including progress notes within 48 hours; respond to inbox messages within 72 hours.
• Telehealth visits when needed; ensure HIPAA-compliant telehealth practices.
• Maintain familiarity with MH/SA pharmacology; identify opportunities for BH treatment.
• Travel locally and regionally as assigned; maintain valid WA driver’s license.
